Receive $8 by mail send:
1. your name, address, city, state, zip
2. upc from Orajel overnight cold sore patch, cold sore brush, plaque revealing rinse, or plaque revealing pre-brush gel
3. dated receipt with item rice circled. Refund will not exceed purchase price. receipt must be dated after 10/26/08 and postmarked by 1/31/09
